In accordance with the Veterinary Surgeons Act 1966, no animal can legally receive physiotherapy intervention without written veterinarian consent. For human referrals, a patient consent form can be completed on initial assessment in person by the patient themselves - consent from doctor/GP is not required for human physiotherapy sessions.

For animal physiotherapy referrals, please complete the owner/animal sections of the below referral forms and hand over to your vet to fill out their section. Please ensure referral forms are returned to georgiabrownphysiotherapy@outlook.com prior to your first physiotherapy session.
